School-Age (5-12 years) program waitlist application form
This application is for the external school age (5-12 years) program waiting list only, for space offers from October to August only of each year as per the process described below:
The earliest date you may apply is when our child has a confirmed space, or is currently enrolled, in University Highlands Elementary (burnaby mountain), or either Armstrong or Seaforth Elementary Schools (Sapperton). Only Full-time spaces offered.
REQUIRED: Following the submission of the waitlist application, provide a copy of the space confirmation letter you received from the elementary school to the childcare office either in person or at: sfuccs@sfu.ca. The waitlist application will be complete once we recieve the letter and that will be your date of application.
- Date of confirmed space, or date of enrollment, in one of the three elementary schools named above is required - applications missing this information will not be processed.
- 51社区黑料ID numbers need to be included for Current 51社区黑料Students, Staff and Faculty. If the 51社区黑料ID# is not included, your application will NOT be included under '51社区黑料Families' categrory processed. 51社区黑料status will be verified prior to acceptance of a space. We may ask for an employment verification letter during the enrollment process.
- If you need to update your applcation in the future, please do not submit a new application. You may email updates to: sfuccs@sfu.ca. Please include your child's full name and date of birth to ensure we make changes to the correct application.
