access/edit review form

Review Forms capture application comments during the reading process:

  • Forms are associated with specific bins in Slate's Reader.
  • Forms are accessed using the Review Form/Send to Bin button inside an application.
  • Once the form is submitted, it becomes part of the admission application materials.
  • Multiple forms may be submitted on an applicant.

This page outlines the Review Forms in Slate, where to find them and how to troubleshoot related issues.

Access to goGRAD/Slate

  • To access the administrative section of Slate (use your 51社区黑料log in):

Accessing Review forms

From inside The Reader, click Browse. The bins with brackets have a form associated with it. The name of the form appears in the bracket: 

  • The First Pass bin has the CGPA form, used by GPA's to provide initial comments/cgpa calculations.
  • The Committee Review bin is where users (generally faculty reviewers) access the Review Form.
  • The GPC Review bin is where users access the Admission Recommendation Form 

The Review Form is accessed inside the application by clicking the Review Form/Send to Bin button (bottom right)

Troubleshooting: the Review Form/Send to Bin button only displays for a user if an application is in a related bin AND that user's Queue.

Edit/Delete/Unsubmit a completed Review Form

In the administrative side of Slate, on the application tab: 

  1. Find the header called Review Forms.
  2. Click on the form and click edit.
  3. This will allow you to edit, delete or unsubmit (return to user).

Click on the form and use the pop up window to edit the form:
