Student Exception Form

This request form is to be used by staff for graduate students only. For undergraduate student exceptions, please click here. Undergraduate requests will not be processed through this form.
Student exceptions apply to individual student records and will show on the Academic Progress Report. Once the form has been submitted, a confirmation email will be sent with a summary of the information received. If an exception applies to more than 10 students, an excel file can be submitted to gpsreg@sfu.ca with all the required information. Please direct all questions to: gpsreg@sfu.ca.
NOTE: The "Academic Progress Report" is not the same as the "Graduate Progress Report."
Submission Details
Personal information collection notice: The information on this form is collected under the authority of the University Act (R.S.B.C. 1996, c.468) s. 27(4). It is related directly to and needed by the University for services provided by Graduate Studies. The information will be used to administer graduate program progress.
